DC Fire commander at deadly Metro smoke event not trained

National Transportation Safety Board investigators said the fire commander on the scene of the 2015 deadly smoke incident had only two of four incident command training courses and had not been trained to set up the unified command needed for the response to the L’Enfant Plaza mishap that led to Carol Glover’s death.
